What is the FAME Senior Mobility & Individuals with disability program?
The FAME Senior Mobility & Individuals with disabilities program is a transportation service that operates Monday through Friday and limited availability on weekends. To book a ride, riders must be affiliated with a partnering agency pre-approved by FAME Corporations. All riders must complete a program application and work directly with their resident manager/case worker to book a ride. There are two service offerings available:
Service Offering 1
Agencies will log into a scheduling system to reserve a 20-passenger shuttle for the day. The shuttle will take riders to pre-determined (by the partner agency) locations such as the grocery store, shopping mall, local senior center, etc. upon scheduling, the Agency will provide FAME with the list of requested destinations and will work closely with FAME to schedule the times of pick-up and drop off for each trip. All trips must fall within a 5-mile radius of the agency.
Service Offering 2
Agencies will log into a scheduling system to book the 6-passenger van for individual rides for non-urgent medical appointments. Non-urgent medical appointments are scheduled medical visits, excluding outpatient surgeries, medical procedures, and emergency medical visits.
Additional Service Option
At the time of booking, the agency will determine if the passenger(s) requires a *companion rider and will have the option to request one at the time of booking.
*Companion rider: A volunteer that rides with the passenger(s) to provide aid beyond the curb. This can include walking the rider into their medical appointment or assisting with grocery shopping. This assistance does not include any physical contact with the passenger, we cannot lift, carry, or transfer riders in any capacity.
